This On-the-Job Training (OJT) record has been developed to comply with ANSI Z136.1 (Safe Use of Lasers), ANSI Z136.8 (Safe Use of Lasers in Research, Development, or Testing), & UMN Laser Safety Program.
Instructions: This record must be completed for all laboratory personnel prior to granting independent, unescorted operational access to Class 3B or Class 4 laser systems. Training must be task- and equipment-specific, conducted by an authorized trainer or the Principal Investigator (PI). Retain the completed, signed document within the laboratory safety binder and submit a digital copy to the Health, Safety, and Risk Management (HSRM) Laser Safety Officer ([email protected]).

SECTION 3: INSTITUTIONAL PREREQUISITES
The following core requirements must be completed and verified before physical laser operations commence. Provide the date of completion for each item.
UMN General Laser Safety Training (UHS 303): Verification of online module completion.
Safety in the Research Lab (UHS 660): Verification of online module completion.
Laboratory Safety Plan: Trainee has read and signed the lab’s safety plan.
SECTION 4: STANDARD OPERATING PROCEDURES & HAZARD CONTROLS
The trainer must review, demonstrate, and assess trainee competence in the following administrative and engineering environments. Provide the date of completion for each item.
SOP Review: Trainee has read, understood, and signed the specific system Standard Operating Procedures (SOPs).
Laser Controlled Area (LCA) Access Rules: Correct utilization of lighted warning signs, interlocks, barrier curtains, and authorized entry protocols (ANSI Z136.8 compliance).
System Startup & Shutdown: Execution of sequential power-on, key-control activation, warming phases, and safe shutdown cycles.
Laser Protective Eyewear (LPE) Selection: Active verification of correct Optical Density (OD) and wavelength specifications suited to the system configuration. Inspection for physical scratches/damage.
Beam Control & Paths: Use of beam dumps, enclosures, and cards. Validation that the beam line is positioned below or above standard eye levels. Absence of watches, rings, or reflective items.
SECTION 5: ADVANCED OPERATIONAL & ALIGNMENT PROTOCOLS
Laser alignment accounts for most accidental ocular exposures. Trainees must demonstrate strict adherence to low-power alignment guidelines outlined in the UMN Laser Safety Program and ANSI Z136.1.
Alignment SOP Execution: Safe implementation of the system's low power/co-axial alignment protocols using secondary viewing tools (e.g., IR converters, card indicators).
Optics Manipulation & Mounting: Procedures for mounting, cleaning, or replacing mirrors, lenses, or crystals without introducing stray/uncontrolled specular reflections.
Non-Beam Hazard Mitigation: Demonstration of awareness and mitigations regarding associated system risks: High Voltage (> 50V), Laser-Generated Air Contaminants (LGACs), or cryogenic handling.
SECTION 6: EMERGENCY PREPAREDNESS & INCIDENT MANAGEMENT
Trainees must articulate and execute immediate response measures for nominal and off-nominal laboratory states.
Emergency Power Off (EPO): Physical identification and demonstrated access to the system E-stop or laboratory master power cutoff.
Accidental Exposure Action Plan: Immediate steps following an actual or suspected eye/skin exposure: laser power off, secure lab, seek immediate medical attention (911 or UMN Fairview ER), notify PI and HSRM.
Fire & Utility Failures: Protocol for immediate system isolation during a localized laser fire, facilities cooling loop failure, or building power loss.
SECTION 7: TRAINING AUTHORIZATION & SIGN-OFF
