Acceptable documentation of immunizations/screening?
- Official clinic records. The record must include the following:
- your name
- your date of birth
- the name of provider who administered the vaccination
- the complete date (month, day, year) on which the vaccination was administered
- International Immunization cards
- Records printed from an official state immunization database
Locating your medical document
From clinic or provider: You can request to have your health care provider submit your immunization information directly to Biosafety & Occupational Health using the Authorization for Release of Health Information form.
- Fill out the Authorization Form
- Send completed form to your health care provider
Note: Some providers may take 2 weeks or more to respond to request for records.
From an official state immunization database, such as the Minnesota Immunization Information Connection (MIIC) database: Download the record to your desktop/phone and submit it as a PDF. Screenshots are not accepted because they do not include all required information.
Submitting your medical document to BOHD

Fax: (612) 626-9643. This a confidential fax in a secured location.
In-person: Thompson Center for Environmental Management, 501 23rd Ave SE. Place records in a sealed envelope marked "BOHD" and deliver to the reception desk.
